FAMILY HOSPICE

Family Hospice Foundation is a publicly supported non-profit 501c3 foundation dedicated to promote and support end of life care and services in the community through education, fund-raising and gifting to patients and their families.

Family Hospice Foundation was created by the Board of Directors of Family Hospice and is funded solely through donations of friends and family of loved ones who have experienced hospice and by the generosity and kindness of individuals and businesses in the community.

Our Vision is that no person who has a life-limiting illness will be turned away for any reason. That every person and family facing an end of life illness within our community would be aware of and have access to quality compassionate care and support.

                         Major areas which the Foundation supports:

Funding for indigent hospice care and providing increased access to hospice without regard to age, sex, gender, nationally, race, sexual orientation or ability to pay.

Specific needs that generally fall outside the scope of traditional funding sources such as Medicare, Medicaid and private insurance. These could include things like utility assistance, home repairs or transportation.

Community awareness programs may include written materials, public speakers, classes or seminars that allow the public to have a better understanding of hospice services and issues relating to end of life.

Support of special community based services to hospice patients or professionals in the field of hospice services.

DONATION INFORMATION

The Foundation is a non-profit corporation and donations are tax deductible as allowed by law. We are supported entirely by the generosity of persons like you who believe that hospice services are important and can greatly make a difference in the lives of those dealing with the end of life and their families.
