environment, providing the highest level of care. Our patients come first. Family Hospice is seeking a full time Social Worker (MSW or LCSW)The ideal candidate must: - have prior home care or hospice experience
- be familiar
with Medicare and Medicaid
- have excellent communication skills (written,
verbal, and phone)
- be available 4-5 days per week 8a –
5p
- be highly flexible and able to multi task and change direction
in a fast paced environment.
- be a strong advocate for the company in
the community.
Duties include: - Setting up Medicare and Medicaid applications
- Handling initial patient admission and paperwork
- Facilitating
interaction with patients, families, and facilities (hospitals, nursing homes, assisted living)
